A BIRD EYE VIEW ON THE EFFECT OF TAILA INFILTRATION AND VARTI INSERTION IN THE MANAGEMENT OF BHAGANDARA AND NADIVRANA

ABSTRACT
Bhagandara is one among Ashtamahagadas[1] mentioned by Sushrutha considering its difficult management and high recurrence rate. It can be co-related to Fistula -in -Ano of modern science which is a second highest prevalent disease among all ano rectal diseases. Ksharasutra therapy mentioned by Sushruta in Nadivrana and Bhagandara[2] is a very popular treatment today owing to its minimal invasiveness , less complications and low recurrence rate. Apart from this treatment Sushruta and later classics have described variety of Tailas and Vartis for the management of Bhagandara and Nadivrana. But these procedures are clinically less practiced and results are not well tested. In cases of multiple fistulae with heavy pus discharge, Fistula with blind internal opening, instead of ksharasutra, Taila Poorana and Varti insertion into the track would be the better options. In this way they help to drain the pus, making the internal opening patent and thus promoting easy probing. Hence an attempt has been made to focus on the concepts of Taila Poorana and Varti insertion in the management of Bhagandara and Nadivrana.
