MINIMALLY INVASIVE TESTICULAR SPERM RETRIEVAL IN NON-OBSTRUCTIVE AZOOSPERMIA: A COMPARATIVE SURGICAL REVIEW OF MICRO-TESE AND THE DAR AL BARAA N-TESE TECHNIQUE

ABSTRACT
Surgical sperm retrieval is a fundamental component in the management of men with non-obstructive azoospermia. Microdissection testicular sperm extraction (micro-TESE) is practiced and provides satisfactory retrieval rates; however, it is associated with extensive surgical exposure, prolonged operative time, postoperative fibrosis, limited feasibility for repeat procedures, high cost and needs big instruments like surgical microscope with good training. To address these limitations, a minimally invasive sperm retrieval technique was developed at Dar Al Baraa Medical Center. This paper provides a comparative surgical review of micro-TESE and the Dar Al Baraa N-TESE technique, focusing on operative methodology, invasiveness, tissue preservation, repeatability, recovery time, and practical clinical implications. The Dar Al Baraa N-TESE technique demonstrates several procedural advantages, including reduced tissue trauma, shorter operative duration, rapid postoperative recovery, and the ability to safely repeat the procedure without long-term testicular damage no complicated instruments with long training and low cost. These findings support the role of minimally invasive approaches in optimizing patient outcomes in non-obstructive azoospermia.
